There are important ways in which the exchanges of nurses and patients can be improved, for the benefit of both. Communication between nurses and patient are highly charged— often comforting and rewarding, sometimes upsetting and disappointing reassurance. The significance of nurse-patient communication goes far beyond personal hopes and disappointments, for years of research now proves that these exchanges have far-reaching impact. A patient’s interest in the health care process, willingness to cooperate, knowledge, and health itself can be traced to the character of these exchanges.
It is the intimate moments of connection that can make all the difference in the quality of care and meaning for the client and the nurse. As nurses refine their communications skills and build their confidence, they can move from novice to expert. The first step to becoming a better nurse communicator is to build a sound knowledge base about the essential communication behaviors.
Like many who study health and illness and the process of medical care, we believe that medical care is a social process as well as a technical one. In other words, medical care involves more than correct diagnosis and recommendation of treatment. Social factors precede diagnosis, for they are involved in the very experience and definition of illness and the willingness to seek care. In writing this book, we hope to introduce this important body of research to those who will profit from it most—the nurses and patients who have been its subject.

Nursing care at home is a new field, which is gaining credibility due to increased hospital costs. Home care nurses and home nursing services work together in close collaboration. Home care nurses provide assistance when clients, owing to illness or reduced functional capacity, needs help at home in order to cope with routine daily activities. These services are provided by helpers and practical nurses.
They monitor the clients’ state of health and provide guidance and advice about services. In many cases, services are also provided in the evening and on weekends. Home nursing services are used by people of all ages, though the proportion of older people is increasing. For this purpose separate personnel are employed who have trained as public health nurses, specialized nurses, registered nurses or practical nurses. The home nursing services include giving care, taking samples and performing tests. Today even demanding nursing care can be given at home, for many people want to live at home until the end of their life. Home nursing services also include support for family members.
In some countries, home help services and home nursing have been combined to form home care units. Home nursing service has also developed into one link of the care chain, the other links being primary health care, specialized health care, the hospital and the clients’ home. For example, care for an older person with a broken hip is oriented towards home nursing services and care at home rather than towards hospital care.
